As a Clinical Manager, I have responsiblities including supervision of a team of nurses and other field clinicians, facilitate new hire orientation and training of new clinicians. I am also responsible for coordination and/or delivering in-services for field staff, so that they are empowered to provide exceptional care to our hospice clients.
AS an RN hospice case manager, I am responsible to make home visits to clients in designated geographic territories. My assigned duties include administration of medications, wound care, and procedures related to comfort care. I am responsible for monitoring clients conditions, and reporting such changes to clinical or client services manager. I am also responsible for following up with, executing, and properly documenting doctor's orders. As case manager, I routinely perform client assessments. I also supervise home health aides and licensed practical nurses.
As Resident Quality of Care Coordinator, I was responsible for assessing ways in which the facility could improve Quality Measure scores. I was also responsible for leading performance improvement projects, i.e. infection control, pain control, and foley catheter utilization.
As assistant manager, I was responsible for the daily operations of two units (53 beds). I supervised the nursing staff (RN's, LPN's, CNA's and patient safety attendants); ensuring that the best care was provided to our patients. In addition to facilitating patient movement in a timely manner (i.e. admissions, transfers, and discharges), I was tasked with appropriate scheduling of staff, morning huddles, patient satisfaction rounding, payroll maintenance, ensuring that we were meeting quality measures, facilitating meetings for CNA's and maintaining staff discipline. When the need arose, such as a call out or filling in for employees on vacation, I would regularly fill in as house supervisor on off-shifts.
